If you are required to furnish a Statutory Declaration (SD) to obtain consent from your mahram allowing you to perform the Umrah, please contact any Commissioner for Oaths for this purpose.
Alternatively, you can contact any of the following Commissioners for Oaths to arrange for the SD to be made. Only the person giving consent need come, bringing his NRIC or Passport and the Name or Names and NRIC / Passport Numbers of the person / persons who are performing the Umrah.
Please make an appointment with the respective office of the Commissioners before proceeding to make the SD to ensure that a Commissioner is available.
Please bring the completed SD to our office to enable us to process your Umrah visa application.
